TeleMedCare’s TMC Care solution is designed to manage chronic disease in nursing homes, residential aged care facilities and learning disability units delivering significant improvements in the quality of clinical care.

Worldwide increases in the incidence of chronic disease, ageing populations, shortages of medical staff and demand to increase the quality of care have combined to change the requirements of the residential care facility. The acuity of patients entering residential care is increasing whilst the number of Registered Nurses is falling. Staffing of residential care facilities with qualified Nurses is increasingly difficult. Residents in care are becoming increasingly frail.

TMC Care is a clinically excellent solution that empowers carers in residential care facilities to deliver higher quality healthcare and facilitates communication between the residential care nurses, GPs and pharmacists. TMC Care contributes to better management and better outcomes for residents through earlier detection of adverse trends, improved diagnosis and better follow up.

TMC Care is designed for monitoring the health of a fixed cohort of patients requiring regular monitoring, so it can be used effectively in not only aged care facilities, but also other multiple occupancy premises where there is a clinical monitoring requirement, such as prisons. TMC Care enables the residents' care team to take a full range of vital signs measurements including ECG, Blood Pressure, Weight, Spirometry, Temperature, Pulse Oximetry and, optionally, Blood Glucose. Patient Health Questionnaires may also be taken by the care team. All data can be viewed centrally at the care facility or remotely by authorised members of a wider clinical team, including GPs, nurses and pharmacists. TMC Care enables high levels of clinical care, with high levels of flexibility to ensure that the clinical monitoring regime matches the specific requirements of the residents. Remote consultation is facilitated by video conferencing and messaging capabilities.
