TMC_Home

TeleMedCare's TMC Home solution is designed to manage chronic disease in the home for individuals who are at high risk of admission to hospital.

The worldwide increase in the incidence of chronic disease, ageing populations and shortages of medical staff dictate that telehealth technologies are leveraged to care for patients in their own homes.

TMC Home is a clinically excellent solution that bridges the clinical information gap of a traditional care regime where vital signs may only be measured on a clinician home visit, a trip to the GP surgery or, all too often, an emergency visit to hospital.

TMC Home allows patients at home to take a full range of vital signs measurements including ECG, Blood Pressure, Weight, Spirometry, Temperature, Pulse Oximetry and, optionally, Blood Glucose. Patients also use health questionnaires and health diaries; a comprehensive set of health questionnaires as well as health education material may also be delivered through TMC Home. TMC Home has video and messaging capabilities, enabling direct communication between the care team and patient. TeleMedCare's Data Services ensure all data can be viewed remotely by authorised members of the patient's care team. Carers can access full longitudinal data for their patients, and use Decision Support capabilities to assist in diagnosis.

TMC Home has been designed to be easy to use with simple, intuitive graphical interfaces. After initial training, patients of all ages typically show excellent compliance with their monitoring regime.

Typically patients will have Congestive Heart Failure (CHF), Chronic Obstructive Pulmonary Disease (COPD) or other chronic diseases, and prior to using TMC Home may have experienced frequent emergency hospital visits, and anxiety driven by a lack of access to the tools and information to manage their own condition. TMC Home enables patients to manage their own health, as they learn more about their own condition through the use of TMC Home, with the confidence that their care team is also able to review vital health data remotely on a regular basis.
